Recent data do not support that nitrofurantoin and alcohol causes a disulfiram-like reaction or other adverse reactions/toxicities. Historical studies have suggested that alcohol use with nitrofurantoin resulted in a disulfiram-like reaction (40,–42). Likewise, a study found that alcohol did not cause a disulfiram-like reaction with nitrofurantoin in volunteers (44). Alcohol may adversely affect the PK of erythromycin and may increase blood alcohol levels. Toxicity data are limited to a single case that cannot be clearly attributed to the combination of ciprofloxacin and alcohol.

The efficacy of azithromycin was unaffected by alcohol, as survival rates were similar in both alcohol-fed rats and control groups in the pneumococcal pneumonia rat model (13). Alcohol did not affect the efficacy of ceftriaxone in a pneumonia model; survival rates were similar in alcohol-fed and control rats (13). To our knowledge, there are no data available on the efficacy/toxicity or ADR of penicillins.
Levaquin (Levofloxacin) and Alcohol
Although this is reported to be rare (3 per 100,000 cases), I have personally seen more cases than I would expect to with those numbers. Most people recover in two months, but some cases were reported to last between one to two years. Modest alcohol consumption does not necessarily limit the effectiveness of the antibiotics you are taking. Instead, it can slow down how fast you recover from your illness. Levaquin and alcohol might not be a life-threatening combination at first, but they can lead to dangerous complications.

You may need to wait at least 48 to 72 hours after finishing your course of antibiotics before consuming alcohol. Antibiotics are medications prescribed to help treat certain bacterial infections. Alcohol intake has been reported to diminish the antimicrobial effect of doxycycline. Levofloxacin, moxifloxacin, and trovafloxacin had improved efficacy in alcohol-fed rats compared to alcohol-free rats (24). All antibiotics were equally effective at improving survival and had improved efficacy in alcohol-fed rats. Control groups had higher mortality rates that were thought to be due to higher protein binding in the absence of alcohol.
Taking metronidazole with alcohol or propylene glycol (found in some foods, medicines) may result in a reaction called a “disulfiram-like reaction”. Isoniazid is often listed as an agent that can alcohol and anxiety cause a disulfiram-like reaction with alcohol due to its inhibition of aldehyde dehydrogenase (106,–108). Symptoms include headache, palpitations, sweating, flushing, and hypotension (109, 110).

High-tyramine alcohols include home-brewed beer, beer on tap, Korean beer, and vermouth. Most bottled beers are lower in tyramine, but it’s always best to ask a healthcare provider if it’s safe to consume alcohol on these antibiotics. These drugs are often reserved for illnesses like community-acquired pneumonia and severe skin and bacterial infections after other antibiotics have not worked. Some antibiotics when mixed with alcohol can lead to side effects like nausea, vomiting, stomach pain, flushing, and liver damage. Alcohol can also affect how some antibiotics are metabolized (broken down) in the body for elimination.
Alcohol interferes with some antibiotics in different ways, which may alter the effectiveness or side effects. Scientists have linked heavy and binge drinking with an impaired immune system. People with an impaired immune system have a higher risk of infection. In 2018, the Food and Drug Administration (FDA) published updated safety warnings on all fluoroquinolone antibiotics. Do not take other medicines unless they have been discussed with your doctor. This includes prescription or nonprescription (over-the-counter [OTC]) medicines and herbal or vitamin supplements.
You should also avoid taking Levaquin with alcohol, as its effects can increase, causing dizziness and impaired judgement. Despite these limitations, the findings of this review bring into question many of the conventionally accepted alcohol-antimicrobial interactions. While alcohol use should still be avoided with certain antimicrobials, explainer how do drugs work use with select others appears to be acceptable. Despite an FDA warning, we were unable to identify published data that demonstrate an increased risk with concomitant ethambutol and alcohol use.
